We’re making changes to our chronic disease review system
From spring 2026 we are moving to a “birthday‑month” recall system. This means your long term condition review will now take place once a year during your month of birth.
What this means for you:
- your next review may be slightly earlier or slightly later than usual as we align everyone to their birthday month
- after this transition period, you will have one clear, predictable month each year when you will be invited for your review
Why we are making this change:
- improves continuity and consistency – patients know exactly when to expect their review each year
- reduces multiple appointments as, where possible; we will combine checks into a single annual appointment
- spreads demand more evenly throughout the year, helping us offer appointments more promptly
- improves patient safety by ensuring reviews are completed on time
- makes the system easier for patients and the practice to manage
We appreciate your understanding as we make this transition. If your conditions require more frequent monitoring, we will continue to provide this as usual.
