Changing your contact details
To update details like your name, address, phone numbers or email address:
- fill out a change of contact details form
- phone or visit the surgery
Change of address
If you have moved to a new address, your new address will need to be in our catchment area to stay registered with our surgery. If you are unsure please ask at reception and they will be able to help you.
Changing your date of birth
You will need to show us proof that you have changed your date of birth. This could be a birth certificate or other legal document.
